National Health Foundation is a team of ‘upstreamists’ who believe that health can be improved by addressing the root causes of health disparities.
Pathway Recuperative Care is our response to the critical need for a continuum of care for individuals experiencing homelessness who have been released from the hospital after an acute health event. We provide services to both hospital partners and individuals experiencing homelessness. This allows Pathway Recuperative Care to have a two-fold benefit: saving our hospital partners immediate dollars while linking individuals to the necessary services to reach their highest level of health.
